GPs usually care for their patients for an extended period of time, therefore, requests to not only discontinue a patient’s treatment but to assist a patient in a suicide are likely to create intensely stressful situations for physicians. However, in order to ensure the best patient care possible, the competent communication about the option of physician assisted suicide as well as the assessment of the origin and sincerity of the request are very important. This is especially true, since patients’ requests (...) for PAS can also be an indicator for unmet needs or concerns. Twenty-three qualitative semi-structured interviews were conducted to in-depth explore this multifaceted, complex topic while enabling GPs to express possible difficulties when being asked for assistance. The analysis of the gathered data shows three main themes why GPs may find it difficult to professionally communicate about PAS: concerns for their own psychological well-being, conflicting personal values or their understanding of their professional role. In the discussion part of this paper we re-assess these different themes in order to ethically discuss and analyse how potential barriers to professional communication concerning PAS could be overcome. Advance directives are written documents that give patients the opportunity to communicate their preferences regarding treatments they do or do not want to receive in case they become unable to make decisions. Commonly used pre-printed forms have different formats. Some offer space for patients to appoint a surrogate decision maker, and/or to determine future medical treatments and/or give a statement of personal values. So far it is unknown which forms GPs preferably use and why they decide to do so. 23 (...) semi-structured interviews with GPs were analysed using content analysis. Interviewees mainly use short templates and medium length templates with checkboxes to indicate patients’ preferences in regards to life prolonging measures. Especially when patients faced the progression of a disease, participants use the latter version. Only then, the interviewees remarked, patients are capable to rate concrete situations reliably. GPs also realize the importance of the verbal assessment of patients’ preferences; however they rarely keep a written form of the conversation. Some GPs hand out one or more templates and ask their patients to read and think about them at home with the option to talk to them about it later on, while others prefer their patients to fill them out alone at home. Regardless of template usage, most GPs emphasize that ADs require regular updates. GPs tend to see standardized advance directives mainly as a tool to start a conversation with their patients and to identify their real preferences and values. When the patient is still not facing the progression of an already existing disease it could be sufficient to only appoint a surrogate decision maker instead of creating a full AD. However, in all other situations, appointing a surrogate decision maker should be backed up by a written statement of a patient’s general values. Patients and their relatives should always have the opportunity to ask their GP for medical advice when drafting an AD. BackgroundThe proper and ethical inclusion of PWLHIV and their young children in research is paramount to ensure valid evidence is generated to optimize treatment and care. Little empirical data exists to inform ethical considerations deemed most critical to these populations. Our study aimed to systematically review the empiric literature regarding ethical considerations for research participation of PWLHIV and their young children.MethodsWe conducted this systematic review in partnership with a medical librarian. A search strategy was designed and performed within the following (...) electronic databases: Ovid MEDLINE, Embase and CINAHL. We screened titles and abstracts using the following inclusion criteria: (1) a study population of PWLHIV or children under 5 years of age; and (2) collection of qualitative or quantitative data regarding ethics of research participation. Excluded were reviews, commentaries, policy statements, clinical care-related ethics concerns, abstracts, case studies, or studies unrelated to HIV research. Studies were appraised for quality, data were extracted, and studies were qualitatively analyzed using a principle-based ethical framework within the Belmont Report.ResultsOf the 7470 titles identified, 538 full-text articles were reviewed for eligibility and only three articles met full criteria for inclusion within this review. While we allowed for inclusion of studies involving young children born to mothers with HIV, only articles focused on PWLHIV were identified. Within the results of these studies, four themes emerged: (1) adequacy of informed consent; (2) consideration of paternal involvement; (3) balancing risks; and (4) access to research and treatment. A strength of this review is that it included perspectives of international research investigators, community leaders, and male partners. However, only two studies collected empiric data from PWLHIV regarding their experiences participating in researchConclusionResearchers and funding agencies should be aware of these considerations and appreciate the value of and critical need for formative research to ensure clinical trials involving PWLHIV promote ethical, well-informed research participation and, ultimately, improve care outcomes. Background Palliative care is an integral part of health care, which in term has become increasingly technologized in recent decades. Lately, innovative smart sensors combined with artificial intelligence promise better diagnosis and treatment. But to date, it is unclear: how are palliative care concepts and their underlying assumptions about humans challenged by smart sensor technologies (SST) and how can care benefit from SST? -/- Aims The paper aims to identify changes and challenges in palliative care due to the use of (...) SST. In addition, normative guiding criteria for the use of SST are developed. -/- Methods The principle of Total Care used by the European Association for Palliative Care (EAPC) forms the basis for the ethical analysis. Drawing on this, its underlying conceptions of the human and its socio-ethical aspects are examined with a phenomenological focus. In the second step, the advantages, limitations, and socio-ethical challenges of using SST with respect to the Total Care principle are explored. Finally, ethical-normative requirements for the application of SST are derived. -/- Results and Conclusion First, SST are limited in their measurement capabilities. Second, SST have an impact on human agency and autonomy. This concerns both the patient and the caregiver. Third, some aspects of the Total Care principle are likely to be marginalized due to the use of SST. The paper formulates normative requirements for using SST to serve human flourishing. We report specific heat and μSR measurements on Th and/or B substituted UBe13. The specific heat data show that either Th or B substitution reduces the Kondo temperature TK and increases the entropy at the superconducting transition by almost 20%, indicating an enhanced density of states. However, whereas μSR shows clear evidence for magnetic correlations for Th substitutions, no magnetism is observed for B substitutions. The enhanced specific heat jump in the B-substituted material is associated with a change in the (...) superconducting properties as TK is reduced. (shrink)

In this paper, I investigate the relation of Kant's theory of biology to epigenetic accounts of organic generation and development. In the literature, a dispute about similarities between Blumenbach's epigenetic account and Kant dominated the debate for many years (see Lenoir 1980, 1981, and 1982, 17–34, Richards 2000; 2002, 207–37; Look 2006, and van den Berg 2009). Some more recent interpreters claim that Wolff's, more than Blumenbach's account plays the pivotal role in the development of a vitalistic conception of epigenesis (...) in Kant (see Dupont 2007 and Huneman 2007). Although I myself hold the view that Kant's position contains preformistic and epigenetic characteristics, in the current paper I focus solely on an investigation of epigenetic elements in Kant's account and compare them to the corresponding epigenetic elements in Wolff's theory. Section I of the paper is devoted to an analysis of Wolff's most important epigenetic theorems: the notion of the essential power (vis essentialis) and the conception of the part-whole composition of organized matter. Although Wolff describes the essential power vitalisticly, as a principle of life, he understands it as the cause of mechanical motions explaining the generation, nourishment, and the growth of an organism. Wolff's model of the part-whole composition of organic matter is subtle, but committed to fundamental mechanistic assumptions, such as that the organism as a whole is composed of inorganic parts. In section II, I analyze the corresponding elements in Kant's theory: the notion of the formative power and the conception of the whole-part composition of organized materials. Kant describes the formative power as a principle that causes the purposive form of an organized being such that matter and mechanism are the means to the purpose of the being as their end. The purpose of the whole is a functional unit which is in principle superior to the form and matter of the subordinate parts. The parts are combined into such a whole in being mutually cause and effect of each other and in being related to the superior whole. In section III, I respond to the debate in the literature. Against Dupont (2007) and Huneman (2007) I argue that, according to Wolff, the vis essentialis accounts for mechanic effects in matter, whereas, according to Kant, the formative power explains the intentional order (form, end, purpose) of an individual organized being, its parts, and its species. Since this view is closer to Blumenbach than to Wolff, the ongoing comparison between Kant and Blumenbach in the literature is justified. However, the emphasis on the specific part-whole composition that Kant considers to be the determining feature of an organized being can be found only in Wolff and not in Blumenbach—though Wolff and Kant describe it in opposing ways. This increases the systematic importance of Wolff for Kant. Thus, a fresh look on the historical debate is required. (shrink)

In a first step, I will try to analyze the nature of the primal plant and its organ, the leaf. Is the primal plant and its organ, the leaf, an idea or does it consist in matter? If it is an idea, is it the idea of all plants (...) (the plant), the idea of a particular species of plants, or the idea of the individual plant? If it is matter – which kind of matter? In a second step, I will analyze the generic nature of the primal plant and its organ, the leaf. How does the metamorphosis of plants proceed? Suppose the leaf is A and the parts of the plants are B, C and D. Does A transform into B, C, D by a process from A to B to A, from A to C to A, and from A to D to A; or does A transform into B, proceed from B as B to C, and proceed from C as C to D; or does A transform into B, while remaining at the same time A, and proceed from B as A to C, while remaining at the same time A, and proceed from C as A to D, while remaining at the same time A? Which of these models explains our experience of plant generation and growth and allows a meaningful interpretation of Goethe's mysterious claim that 'All is leaf'? (shrink)

Since uncertainty is a major challenge in medicine and bears the risk of causing incorrect diagnoses and harmful treatment, there are many efforts to tackle it. For some time, AI technologies have been increasingly implemented in medicine and used to reduce medical uncertainties. What initially seems desirable, however, poses challenges. We use a multimethod approach that combines philosophical inquiry, conceptual analysis, and ethical considerations to identify key challenges that arise when AI is used for medical certainty purposes. We identify several (...) challenges. Where AI is used to reduce medical uncertainties, it is likely to result in (a) patients being stripped down to their measurable data points, and being made disambiguous. Additionally, the widespread use of AI technologies in health care bears the risk of (b) human physicians being pushed out of the medical decision-making process, and patient participation being more and more limited. Further, the successful use of AI requires extensive and invasive monitoring of patients, which raises (c) questions about surveillance as well as privacy and security issues. We outline these several challenges and show that they are immediate consequences of AI-driven security efforts. If not addressed, they could entail unfavorable consequences. We contend that diminishing medical uncertainties through AI involves a tradeoff. The advantages, including enhanced precision, personalization, and overall improvement in medicine, are accompanied by several novel challenges. This paper addresses them and gives suggestions about how to use AI for certainty purposes without causing harm to patients. (shrink)

Higgins-Biddle's new edition of the work Locke published anonymously in 1695 is another fine entry in the Clarendon series. It (...) is the first critical edition and the only one to include the revisions Locke made in a personal copy. Higgins-Biddle provides a set of Locke's manuscripts related to the text (including manuscript indices), a table of Biblical references, and a bibliography. In his meticulously researched introduction, Higgins-Biddle sets out the background and main themes of The Reasonableness by considering the controversies that surrounded it, particularly the accusations of Deism, Socinianism, and Hobbism.Locke's title is apt to mislead contemporary readers by encouraging them to expect a rationalistic defense of religious belief. What we have instead is a treatise largely devoted to discovering the basic beliefs one must hold in order to be a Christian. Christianity is 'reasonable' according to Locke because its fundamental doctrines are perspicuous and accessible to all, not because it is in every respect rationally demonstrable. Locke argues that belief in two propositions, viz., that God exists and that Jesus Christ is the Messiah, is sufficient to make one a Christian. Early critics, as Higgins-Biddle shows, mistakenly took this to mean that other doctrines, especially that of the Trinity, formed no part of Locke's brand of Christianity. Instead, Locke suggests that although belief in the fundamental doctrines is sufficient to make one a Christian, one is subsequently obligated to receive "all the parts of Divine Revelation, with a docility and disposition prepared to imbrace" (169). Locke grants, however, that a Christian may be ignorant of or even disbelieve a large portion of scriptural truths. His failure to specify which particular 'truths' he had in mind seems to have led to the charge of Socinianism, which persists to the present day.It is not clear what this charge amounts to, and Higgins-Biddle does a fine job of sorting through the religious controversies of Locke's age. One core plank of Socinianism or Unitarianism seems to have been the denial of the Trinity. Nowhere in The Reasonableness does Locke so much as mention that doctrine; however, his position does make it a secondary one, since assent to it is not required of all Christians. What is most important here is the continuity between Locke's project in this work and his thoughts on toleration. The chief aim of those who, like Locke, advocated the 'way of fundamentals' was to achieve toleration for non-conformist religious sects and to make possible a more harmonious and unified Church of England.Locke is often regarded as a founding father of English Deism, a view Locke understood as entailing the denial of all religious claims except those supported by natural theology. But Locke himself saw The Reasonableness as partly directed against the temptation to reject the claims of revelation. Such claims can be objects of faith, accepted by virtue of Biblical authority. Opposition to Deism is particularly evident in Chapter XIV, where Locke argues for the necessity of Christ as saviour. Education is important in shaping professional identity, including how one approaches norms and normalisation. In the analysis presented in this study, nursing students' own constructions of norms and normality from the outlook of their education are highlighted and problematised. To deepen the understanding of these matters, the aim of this study was to explore constructions of norms and normality among students in nursing education. Students studying in a nursing department at a Swedish university college were approached and asked to consider (...) open survey questions targeting their views on norms and normality; 154 of them replied. After a discourse analytic approach to the data, we could see how the students constructed norms and normality as (a) instrumental instructions, consisting of easy‐to‐digest statements grounded in the profession's obvious moral and ethical values, (b) limiting and frustrating obstacles for personal freedom that were important to challenge, (c) rules to be obeyed for the stability of society and (d) a matter of reflection, with each individual being responsible for understanding differences in norms, perspectives and opinions. We conclude that nursing education would benefit from norm‐critical perspectives, problematising students' own positions to norms, power and privilege. (shrink)

Although a fascination with language is a familiar feature of 20th-century empiricism, its origins reach back at least to the early modern period empiricists. John Locke offers a detailed (if sometimes puzzling) treatment of language and uses it to illuminate key regions of the philosophical topography, particularly natural kinds and essences. Locke's main conceptual tool for dealing with language is 'signification'. Locke's central linguistic thesis is this: words signify nothing but ideas. This on its face seems absurd. Don't we need (...) words to signify things as well? But its very absurdity – our inclination to dismiss Locke as a 'linguistic idealist'– should signal to us that we have not yet understood Locke. Doing so must begin with an analysis of signification. Each of the three main interpretations on offer allows Locke to escape the charge of linguistic idealism, although they do so in very different ways. Locke's text also offers an influential account of linguistic particles, words like 'is', 'and' and 'if'. These signify, not ideas, but acts of the mind. These acts can either take place within a proposition, uniting its constituent ideas into a thought that admits of a truth-value, or they can take propositions as their objects, in which case they express attitudes like doubt, assertion and so on. Even this seemingly innocuous sketch of Locke's view is controversial, and many writers, from J.S. Mill onwards, have argued that Locke cannot make sense of propositional attitudes. Apart from the intrinsic interest of these questions, understanding how Locke thinks language works is a prerequisite for understanding his arguments against scholastic essentialism. Otte :165–177, 2009) and Pruss :400–415, 2012) have produced counterexamples to Plantinga’s famous free will defence against the logical version of the problem of evil. The target of this criticism is the possibility of universal transworld depravity, which is crucial to Plantinga’s defence. In this paper, we argue that there is a simpler and more plausible free will defence that does not require the possibility of universal transworld depravity or the truth of counterfactuals of creaturely freedom. We assume only (...) that libertarianism is possibly true and that God’s existence is consistent with the existence of free agents who never go wrong. We conclude the paper by explaining how our defence may be able to succeed without assuming, in a way that is consistent with compatibilism. (shrink)

This compact profile by the co-translator of Sein und Zeit is one in a series of introductory studies of major contemporary theologians and philosophers who have influenced theology. This study of Heidegger is a remarkably lucid general introduction to his thought. Macquarrie considers in turn Heidegger's "life", "thought", and "significance." Macquarrie accepts for the most part Heidegger's own self-interpretation of his development--that his thought represents a continuous progressive path, all along guided by the problem of being. As one might expect, (...) there are a number of illuminating comparisons between Heidegger and other theological literature, including gnosticism, Barth, Bultmann, Tillich, and Heinrich Ott. The account of Heidegger on God is among the most meritorious features of the book. The references to Heidegger are to the English translations. There is no bibliography, but there is an excellent glossary which explicates clearly, simply, and accurately many important words in Heidegger's vocabulary.--J. D. C. (shrink)

Kockelmans’ book is an anthology of readings which he has collected together and in most cases translated. Eight of the eleven selections have already been published—although not all have appeared in English. Part I contains two essays with no evident connection—one by Kockelmans and a 1961 article from a Dutch periodical by Jan Aler on the view of language in Being and Time. Part II is the heart of the book and contains the translated texts of the six lectures given (...) at the Colloquium at Penn State in 1969 on "Heidegger’s Conception of Language," of which Kockelmans was the director. It also includes the panel discussion among the participants: Walter Biemel, Otto Poeggeler, Henri Birault, Heinrich Ott, Werner Marx and Kockelmans. These papers are extremely high level Heideggerian scholarship and very penetrating studies. They will repay careful study. Part III contains papers of a linguistic nature by Johannes Lohmann and Erasmus Schoefer. The essay by Schoefer is interesting; it is excerpted from his valuable book Die Sprache Heideggers which studies Heidegger’s own use of the German language. This is a worthwhile collection of studies which is enhanced by a bibliography and index.—J. D. C. (shrink)

Michel Foucault (1926-1984) is considered one of the most important thinkers of the second half of the 20th century. In French philosophy, as a companion of structuralism and critical theory, he outlined his own theoretical field. The purpose of the present work is to prepare for the reading of Foucault texts, which are mostly translated into Slovene.

On a currently popular reading of Locke, an idea represents its cause, or what God intended to be its cause. Against Martha Bolton and my former self (among others), I argue that Locke cannot hold such a view, since it sins against his epistemology and theory of abstraction. I argue that Locke is committed to a resemblance theory of representation, with the result that ideas of secondary qualities are not representations.

Purpose of the article One of the approaches how to create a concept of a brand is a form of identifying association network in the mind of the consumer and creating semantic maps composed of all associations that are largely shaped by cultural values of consumers. Methodology/methods In the first phase, the author of the study was detecting the associations connected with the Mexican brand alcoholic beer Corona Extra using focus group with a sample of 15 respondents. In the second (...) phase, she analysed the data found in the context of Slovak cultural values and compared them with the results of the secondary research. Scientific objective The article highlights the importance of understanding cultural values of the target market for the management and brand communication across cultures. The main objective of the survey was to determine how the Slovak respondents perceive selected sample of global brand, to compare the findings of a secondary survey conducted at German and Spanish students, and in the final phase of the research to analyse the data found in the context of the cultural dimensions of respondents. Findings On one hand, the associations formed by consumers about Corona Extra mutually differ depending on the countries surveyed. On the other hand, they correspond to the cultural values of the nation, and thus confirm the strong influence of the cultural aspects of the brand perception. Conclusions The presented article confirms, that there will always be an open space for the extensive research in global marketing and cross-cultural differences in consumer behaviour. Exploring cultural aspects of communication of global brands will benefit not only in terms of increasing the efficiency of advertising campaigns, but also for the mutual knowledge and understanding between cultures. Disputes over territory are a major contributing factor to the disruption of international relations. We believe that a cumulative, integrated, and continuously updated resource providing information about such disputes in an easily accessible form would be of benefit to intelligence analysts, military strategists, political scientists, and also to historians and others concerned with international disputes. We propose an ontology-based strategy for creating such a resource. The resource will contain information about territorial disputes, arguments for and against claims pertaining to sovereignty, (...) proffered evidence for such claims, political and military motives (overt or hidden), and associated conflicts. Our approach is designed to address several issues surrounding the representation of geopolitical conflict, including the tracking and individuation of disputes and the validation of disseminated information. In 1985, Swiss legal philosopher Walter Ott visited Herbert Hart in Oxford and made this record of their meeting, which casts novel light on some of Hart’s ideas. Ott engaged Hart in a fresh encounter with the legal philosophy of Gustav Radbruch, particularly Hart’s and Radbruch’s reasons for a minimum content of justice in law. They also discussed the grudge informer, state responsibility under laws of an earlier régime, and questions of the definition and falsifiability of legal theories. Hart surprisingly (...) reported Hans Kelsen as stating, when they had met at Berkeley in 1961, that Hume (and not Kant, and certainly not Wittgenstein) was the greatest philosopher who had ever lived. How language works — its functions, mechanisms, and limitations — matters to the early moderns as much as it does to contemporary philosophers. Many of the moderns make reflection on language central to their philosophical projects, both as a tool for explaining human cognition and as a weapon to be used against competing views. Even in philosophers for whom language is less central, we can find important connections between their views on language and their other philosophical commitments.

Food security is becoming an increasingly relevant topic in the Global North, especially in urban areas. Because such areas do not always have good access to nutritionally adequate food, the question of how to supply them is an urgent priority in order to maintain a healthy population. Urban and peri-urban agriculture, as sources of local fresh food, could play an important role. Whereas some scholars do not differentiate between peri-urban and urban agriculture, seeing them as a single entity, our hypothesis (...) is that they are distinct, and that this has important consequences for food security and other issues. This has knock-on effects for food system planning and has not yet been appropriately analysed. The objectives of this study are to provide a systematic understanding of urban and peri-urban agriculture in the Global North, showing their similarities and differences, and to analyse their impact on urban food security. To this end, an extensive literature review was conducted, resulting in the identification and comparison of their spatial, ecological and socio-economic characteristics. The findings are discussed in terms of their impact on food security in relation to the four levels of the food system: food production, processing, distribution and consumption. The results show that urban and peri-urban agriculture in the Global North indeed differ in most of their characteristics and consequently also in their ability to meet the food needs of urban inhabitants. While urban agriculture still meets food needs mainly at the household level, peri-urban agriculture can provide larger quantities and has broader distribution pathways, giving it a separate status in terms of food security. Nevertheless, both possess potential, making them valuable for urban food planning, and both face similar threats regarding urbanisation pressures, necessitating adequate planning measures. (shrink)

Second are teachers of modern philosophy. The back-and-forth among these figures makes for a refreshing change from the massive, often self-contained works that characterize much of the rest of such a (...) course. This article seeks to show the interaction between cultural and anatomic evolution in the birth and differentiation of cultures and languages. The diversity which characterizes our world does not preclude logical regularities due to the coherence of the human mind, which evolved slowly through the paleontological phases of its emergence over millions of years. The anatomical retroaction of the hominids is analyzed to show that, in the long run, anatomy reflected ‘cultural selection’.
